I, the below-signed PARENT or GUARDIAN of the below-named CAMPER,

understand and assume all of the risks connected or related to such activity and fully release and discharge

the Board, Staff, and Membership of Wonder Book Camp, and all other individuals participating in any way

with Camp from any liability in any way associated with any occurrence during camp.
